DUTIES AND RESPONSIBILITIES

1. Provide first-line point of contact for all patients coming into the Department. In the absence of the Secondary Breast Cancer Nurses apply counselling/communication skills to deal with distressed patients/carers or bereaved relatives, either in person or on the telephone. Assess the situation and exercise judgement on appropriate action and give non-clinical advice, identifying issues that require the urgent attention of the Secondary Breast Care Nurses.

2. Triage telephone and email enquiries, referring complex decisions to the team for assessment and review.

3. Coordinate and organize appointments, assessments and investigations as required.

4. Make pre planned telephone calls to patients to enable proactive prevention of crisis approach.

5. Demonstrate the ability to recognize and respond appropriately when faced with a deterioration or emergency situation, alerting the team or enabling rapid response.

6. Provide general information and support about cancer and cancer services to enable navigation of health and social care doing referrals where appropriate.

7. Accept referrals for the Secondary Breast Specialist Nurses from other Health Care Professionals and make referrals on behalf of the secondary breast care nurses to hospital and community teams. Liaise with all grades and disciplines of staff within the Trust, the Community and other hospitals as appropriate.

8. Use of Desk-top Publishing/Advanced computer skills for the production of in-house patient information leaflets, PowerPoint teaching presentations, business cards for the Team.

9. Appropriate use of relevant information technology systems, e.g. Secondary Breast Patient data base

10. Required to work using own initiative, without supervision.

11. Manage own workload and work independently.

12. Organization and administration of Clinics, including ensuring all appropriate referral paperwork is available and typing all related correspondence.

13. Being aware of Trust procedures and policies.

14. Represent the Secondary Breast Cancer Nursing Team as appropriate at Conferences and Open Days.

15. Administrate and accurately facilitate meetings with both nursing team and Secondary Breast Cancer Focus Group.

16. Co-ordinate specific areas of work emerging from the Greater Manchester Cancer in liaison with the Cancer Nurse Specialists and be involved in any long term projects being involved in service development and change.

17. Take the lead in planning and implementation of health and wellbeing days as an ongoing long term project ensuring evaluation is carried out. This will include attending the events, deputizing for presenters, being a safeguarding advocate and managing attendee invitations and lists.

18. Extensive knowledge and understanding of medical terminology required to transcribe patient notes and letters and to be able to accept complex medical referrals to the Secondary Breast Cancer Nursing Service from other Health Professionals. This detailed information must be accurately relayed to the Secondary Breast Specialist Nurses.

19. Required to apply counselling skills to manage upsetting situations over the telephone or when distressed patients and/or relatives come to the Department.

Key Working Relationships

The post holder will build and maintain effective working relationships with:

• The Christie wider breast team both on site and at outreach oncology clinics within Greater Manchester and Cheshire.
• Cancer clinical teams, Macmillan Information and Support Centre and wider clinical / corporate / community network, including MDT Co-ordinators, clinical support services; external statutory / charity / third sector organisations within Greater Manchester and Cheshire.
• People affected by cancer

To undertake any other duties deemed appropriate to the band when requested by Senior Staff

Qualifications

Essential

• GCSE English language and Mathematics
• NVQ Level 3 in health related subject/ equivalent knowledge experience

Desirable

• Willingness to undertake learning and development relevant to role
• Willing ness to undertake communication skills course

Experience

Essential

• Relevant health and social care experience at Band 3 or equivalent
• Experience of multiprofessional working
• Experience in use of data base management
• Experience of using good communication skills
• Understanding a person centred care
• Experience of providing administrative support

• Understanding of Macmillan Cancer support
• Experience in coordinating a patient caseload
• Awareness of national cancer policies
