Job summary

An excellent opportunity has arisen for a motivated and highly enthusiastic Social Prescriber to join our community respiratory team working in the Long Covid Service.

You will work alongside CCICP respiratory colleagues. You will also be an essential part of the weekly MDT to support patients with ongoing symptoms post Covid-19.

Social prescribing empowers people to take control of their health and wellbeing. A Social Prescriber gives time, focuses on 'what matters to the patient' and takes a holistic approach to an individual's health and wellbeing, linking people to sources of support within the local community for practical and emotional support.

The Social Prescribing Link Worker will be embedded within Post Covid Recovery multi-disciplinary team to provide personalised support to individuals to take control of their wellbeing, live independently and improve their health outcomes.

In service training specific to the role will be provided as needed.

Main duties of the job

  • To work collaboratively within the Post Covid Multi-disciplinary team to identify appropriate patients for referral into social prescription.
  • Play an active part within the MDT by offering expertise on personalised support to individuals to take control of their wellbeing, live independently and improve their health outcomes.
  • Arrange 1:1 contact with the patient to discuss the referral and co-produce and personalised support and care plan for their health and well being.
  • Where appropriate, physically introduce people to appropriate community groups, activities, and statutory services, ensuring they are comfortable, feel valued and respected. Follow up to ensure they are happy, able to engage, included and receiving good support.
  • Organise and run peer support groups for Post-Covid patients with support of qualified team members.
  • Develop strong links with a wide range of local VCSE organisations, community, and neighbourhood level groups, utilising their networks and building on what's already available to create a menu of diverse community groups and assets, who promote diversity and inclusion.
  • To build networks with local community resources such as activities and services that may have an impact on health and wellbeing.
