Mid Cheshire Hospital NHS Trust enjoys a proactive and continually developing Colorectal cancer service. We are looking for a Colorectal Advanced Clinical Practitioner to join the team to continue to maintain an exemplary nurse led Colorectal cancer pathway.The post holder will be integral to maintaining and overseeing the Colorectal cancer pathway.

The Colorectal Advanced Clinical Practitioner will provide expert, evidence-based advice and clinical expertise within Colorectal and cancer services, working collaboratively and co-operatively with others to meet the needs of patients and their families. The clinical workload covers all aspect of the rapid diagnosis pathway. Including nurse-led triage assessment to assess suitability for the straight to test pathway, to clinical consultation in the rapid access clinics, onward referrals or discharging back to primary care, with cross cover of the colorectal cancer pathway as needed. They will work alongside the Colorectal Nurse Consultant, the Colorectal Clinical Nurse Specialists and the support team.

A highly visible and proactive leader, working autonomously, under the guidance and supervision of the Colorectal Consultant Nurse, to ensure sustained delivery of high-quality patient care for people at all phases of the Colorectal cancer pathway.

Main duties of the job

The post holder will work as an Advanced Clinical Practitioner. They will be an experienced practitioner with decision making autonomy, responsibility and accountability.

They will demonstrate a high degree of clinical autonomy and complex clinical decisions within their specialist field utilising evidence-based practice. You will contribute to the leadership, development and supervision of others.

The post holder is expected to take responsibility, provide care and make clinical decisions is accordance with agreed clinical pathways of care and proficiency of own skill set.

The post holder will hold a master's qualification in advanced practice.

The post holder must be able to work independently within the four pillars of advanced practice which include clinical practice, management and leadership, education and research.

As an ACP, you will be expected to work alongside the medical and nursing teams to promote the delivery of a high standard of patient centred care within a supported environment.
