Job summary
An exciting 12 month opportunity has arisen for a band 4 Community Stroke Rehabilitation Assistant to work within the Knowsley Community Stroke Rehabilitation service. You will be an integral part of the Knowsley Community CVD Service. Instrumental in assisting the Community Stroke Team to delivery patient rehabilitation programmes to patients registered with Knowsley GP's who have had a CVA. You will provide appropriate support and rehabilitation within the community setting working across a variety of venues and within patients homes. You will work across the disciplines of Physiotherapy, Occupational Therapy, Speech and Language Therapy to development, coordination and delivery a comprehensive programme of Stroke Rehabilitation. This is an exciting opportunity to contribute to peoples recovery and see the impact rehabilitation can have. Delivering healthcare to people within their own homes and experience holistic care delivery within a multidisciplinary team.
Main duties of the job
To provide a service to patients registered with Knowsley GP's who have had a CVA and provide appropriate support and rehabilitation within the community.
- To carry out assessment, treatment, modification, follow up and discharge for a designated caseload working across health and social care boundaries in liaison with qualified staff.
- To actively assist the Community Stroke Team in the development and delivery of effective patient rehabilitation programmes by working across the disciplines of Physiotherapy, Occupational Therapy, Speech and Language Therapy.
- To work with qualified staff in the development of clinical procedures, protocols, and practice to ensure the effective delivery of services to a designated patient population.
- To identify and initiate processes and procedures to ensure the effective management of clerical/ administrative duties.
- To demonstrate initiative in carrying out designated and non- designated tasks as required.
To maintain clinical and non -clinical documentation (using electronic systems) in line with organisational and departmental requirements.
- To accept clinical responsibility for a designated caseload of patients, and to organise this effectively and efficiently with regard to clinical priorities, clinical risk, use of time and staffing levels. To balance patient related and professional demands, ensuring that these remain in accordance with those of the team.
