Job summary

A rare and exciting opportunity has arisen to join the Liverpool Adult Cystic fibrosis (CF) team. We are looking for an experienced and enthusiastic dietitian to join a well-established multi-disciplinary team delivering high quality care to people with CF, on an in and outpatient basis.

We are a large CF centre, caring for 377 adult patients with a catchment area covering the North West, North Wales and the Isle of Man and an international reputation for research.

Main duties of the job

You will work alongside an experienced specialist CF dietitian, within the wider dietetic department. The role requires a motivated and adaptable approach, with an ability to manage a large, complex caseload and strong multidisciplinary working.A keen interest in cystic fibrosis is essential, with experience in nutrition support, weight management, diabetes and supporting patients with a chronic condition. Experience in audit, quality improvement and research is desirable and such projects are encouraged.

JOB SUMMARY

To provide factually accurate, unbiased evidence based nutritional advice appropriate to the needs of the individual. Advice will be provided using a patient centred approach, in the context of multidisciplinary, multiagency delivery plans.

To provide the nutrition and dietetic service as clinical lead to your own specialist area.

This involves:-

1. Personally providing the dietetic service to in-patients and out-patients.

2. Educating/training other members of the multi-disciplinary team in specialist area.

3. Clinical supervision of new members of the dietetic team.

4. Training of Student Dietitians

5. Development and implementation of policies, protocols and guidelines relevant to clinical speciality.

PRINCIPAL ACCOUNTABILITIES

CLINICAL

For each patient, collect all relevant medical, social, biochemical and pharmacological information required to inform practice.

Undertake detailed nutritional assessment using, weight, height, mid upper arm circumference and skin-fold measurements as appropriate.

Undertake dietary assessment, estimating intake of macro and micronutrients as appropriate based on disease state.

Using all information gathered, compile and agree with patient and/or carers a nutritional care plan.

Inform all relevant health care professionals of main aims and objectives of care plan by completing relevant documentation or writing report in medical notes.

Monitor progress on the nutritional care plan according to local standards, amending as necessary to ensure improvement in health is achieved, and informing other health care professionals of any changes.

Establish and maintain effective communication when giving nutrition and dietetic advice to individuals and groups, ensuring technical information is given in a way that is understood.

Communicate with patients and carers to explain principles of nutrition management and its effect on disease progression and outcomes.

Communicate relevant information about patients and their management to relevant clinicians and other appropriate members of the medical staff and health care professional in other NHS Trusts & Community Services.

High level communication skills and the ability to develop a rapport with challenging patients and their carers.

Attend ward rounds, case conferences and ward meetings as required to reinforce importance of good nutrition in patient care and role of Dietitian in multidisciplinary team.

Where appropriate, use advanced communication and counselling skills to assess motivation and overcome barriers to change.

Liaise with relevant staff and contract caterers regarding provision of suitable meals for in-patients. Advise on restrictions and nutritional adequacy as appropriate.

Ensure continuity of patient care by liaising with primary care colleagues and other agencies involved in care of patients at home for example patients discharged on enteral feeding.

Provide cover for absent colleagues thus, requiring knowledge in other areas of dietetics.

Assist wards in maintaining stock of enteral feeds and supplement drinks.

Plan and prioritise own workload using clinical judgement.

MANAGERIAL/LEADERSHIP

Organise cover for absence of members of your team by prioritising and allocating work as necessary.

Regularly review clinical supervision guidelines for own specialist area.

Undertake clinical supervision of other dietitians within the team.

Ensure continuity of patient care by sharing resources and knowledge with primary care colleagues.

Be an enthusiastic member of the senior leadership team within the therapies department.
