Job summary
The post holder will work as a Respiratory Nurse/Physiotherapist assisting in the assessment, planning, delivery and evaluation of evidence-based care for patients with chronic lung disease within a hospital and community setting. Although the role will primarily involve working within the Home Oxygen element of the service, the successful candidate may also be required to review patients in the local AED or ward areas, facilitating an early/supported discharge where appropriate and safe to do so, and following them up at home. In addition, they will be responsible for triaging, assessing and monitoring acutely unwell patients within the home environment to avoid unnecessary admissions, as well as supporting the Consultant/Registrar in the multi-disciplinary clinics.
The service is operational 24 hour a day, 365 days a year, with an overnight on-call rota which the post holder will be required to participate in.
Main duties of the job
Assist the Respiratory Nurse and AHP Specialists in managing patients within the hospital and community setting, providing support to patients allowing them to develop self-management skills
Plan, implement and evaluate care delivery according to changing health care needs and in accordance with service policies
Develop advanced clinical skills in history taking, clinical examination, assessment and/or treatment of respiratory disease
Undertake arterial and/or capillary blood gas sampling in accordance with agreed guidelines and protocols for patient assessments, including oxygen therapy and respiratory management
Order required oxygen therapy, undertake home visits for follow up assessments and make any required changes to oxygen prescription
Work collaboratively and innovatively in order to deliver pioneering developments to enhance patient care within the hospital and community setting
Impart specialist knowledge, advice and support to colleagues, patients, carers and relatives
Input into data collection systems collating information about patients as and when required, facilitating review and audit
Be aware and adhere to NMC guidelines/HCPC Code of Conduct, as well as Trust policies and procedures at all times
