An exciting opportunity has arisen for a permanent Cardiovascular Rehabilitation Team Leader within the existing Knowsley Community Cardiovascular Service.
The post will be based within Knowsley community service currently based at Liverpool Heart and Chest Hospital but providing care across the Borough of Knowsley. You will be working as part of a multidisciplinary team with and excellent motivated experienced Cardiovascular Team.
The services covers a complex approach across multiple co morbidities in Cardiovascular care., ie Heart Failure, Stroke, PVD, Cardiac revascularisation, you will be responsible for delivering a high quality Cardiovascular programme in line with the Commissioned Service.
Main duties of the job
The post holder will be responsible for supporting the co-ordination, expansion, evaluation, and delivery of a menu driven approach support patients manage their long term conditions. They will ensure that a high standard of patient care is provided, and that the principles of evidence based practice and best value are integral to practice and the core business objectives, in improving services for users in line with the patients vision.
The successful candidate will be expected to communicate with a wide range of people, from patients and their families, other health care professionals and external agencies.
Support from the Consultant, Lead Nurse and Matron for Community Services will be given to develop your clinical and leadership skills.

1. JOB SUMMARY
To act as the Lead facilitator in the delivery of high-quality Cardiovascular Rehabilitation in the Knowsley Commissioned Community Services. MAIN RESPONSIBILITIES
To act as a source of expertise on the management of patients within the Cardiovascular rehabilitation service providing advice to primary health care practitioners, consultants and senior health care colleagues.
To provide patients with informed choice regarding the management of their Cardiovascular condition ensuring that patients supported in self-management and long term health gains.
To create a single point of access enabling effective triage and signposting to the most appropriate service(s) for the patient.
To work in collaboration with the Community Services Matron and Senior Nurse in developing a performance framework to ensure that the commissioned Community Service are achieving Key Performance Indicators (KPIs).
To manage submission of national audit data to the National audit of cardiac rehabilitation (NACR)
To suggest ways to improve the infrastructure of the Rehabilitation Service, creating a network of professionals with consistent referral methods, protocols, clinical management and effective communication channels generating a confidence in all users of, and referrers to, the rehabilitation services.
To ensure governance arrangements are in place, robust and consistent across the whole service.
To set the benchmark standard for delivery of Rehabilitation for Knowsley patients.
To operationally manage the Cardiovascular Rehabilitation Team, ensuring training and competency of all team members delivering CVD and respiratory care are up to date and valid.
To enhance skills of established staff to enable a high standard of service in assessment, treatment, education and advice for Knowsley patients.
To act as research and audit lead for the Community Services ensuring that the modernisation agenda is fulfilled and thoroughly evaluated.
To be responsible for managing complex projects and systems change within Cardiovascular Rehabilitation
2. CLINICAL RESPONSIBILITIES
If applicable Registered practitioners who are non medical prescribers are accountable for their prescribing activities by ensuring they are aware of their Professional accountability and any restrictions or special considerations in their prescribing practice
To be professionally and legally responsible and accountable for all aspects of your own work including the management of patients in your care. To ensure that a high standard of clinical care is maintained within the clinical environment/ areas for which you are responsible.
To interpret and analyse clinical and non-clinical facts to form an accurate clinical diagnosis and prognosis for patients with complex problems, which may include the appropriate recommendation of intervention from Primary, Secondary or Intermediate Care and/or other agencies.
To make autonomous decisions on patient care based on current evidence and best practice, ensuring the rapid assessment and acceptance of patients requiring Cardiovascular condition management.
LEARNING AND DEVELOPMENT
To train, supervise and performance manage staff within the multi disciplinary team where appropriate and ensure the clinical competence of staff is maintained.
