Job summary

Ward 1 is a 12 bedded General Medical ward which supports patient flow. The patients on Ward 1 are transitioning from acute care to their discharge destination. Teamwork and person-centred care are at the forefront of our working day.

Main duties of the job

  • To demonstrate up to date knowledge of national and local professional issues and to lead/facilitate the implementation at a local level.
  • To undertake any other duties, required by the Line Manager appropriate to the grade and relevant to the post.
  • To promote collaborative working partnership and effective communication between all members of the multi-disciplinary team.
  • Promote health and wellness through practice and education of patients, relatives, carers and staff.

The Secretary of State has approved the partnership between St Helens & Knowsley Teaching Hospitals and Southport & Ormskirk Hospitals.

From 1st July we have come together as a single organisation under the name Mersey & West Lancashire Teaching Hospitals NHS Trust.

To assist in the setting and monitoring of standards of nursing care within the ward/ department.5. To encourage the participation and involvement of the patient and carer(s) in the assessment, management and evaluation of care.6. To liaise with all relevant disciplines involved with direct care and initiate health promotion and education.7. To work alongside multidisciplinary team in coordinating patient care and rehabilitation.
