Job summary

This post is for a consultant in Geriatric Medicine. This is a whole-time post (can be negotiated) offering an exciting opportunity for a highly motivated Medic to join a dynamic, rapidly expanding team delivering multidisciplinary acute care in Mersey and West Lancashire Teaching Hospitals.

Applicants are required to hold Membership of the Royal College of Physicians (UK or equivalent qualifications), to be fully registered with the General Medical Council, and to be on the Specialist Register or due to obtain their CCT within six months of the date of interview.

Main duties of the job

This is a new consultant post which has been established to support our revamped integrated frailty pathway. This is focused on providing evidence based high quality care of older people with Frailty, incorporating community and hospital services.

You will be joining a dynamic, skilled, and motivated Care for Older Peoples department based at Southport hospital with a commitment to provide a truly integrated high-quality service to our elderly frail population. The successful candidate will work alongside colleagues in a range of duties on the 28 bedded frail elderly short stay (FESS), and any other of the COE wards, as well as providing support to ED and community support to GPs and in reach frailty teams

The post holder will

  • Carry out inpatient elderly care ward and board rounds. (14 Inpatients)
  • Opportunity for Frailty or condition specific outpatient clinics (3 New patients, 4 Follow Up Patients) including a HOT Frailty clinic.
  • Be encouraged to be involved in the Frailty Improvement programme committed to delivering change and improvements within the service, including the development of a Frailty Assessment Hub
  • Ensure that Estimated Discharge Dates are set on admission for inpatients.
  • Take an active role in the development and implementation of the local integrated frailty pathway.
  • Support delivery of national targets (18 weeks, new to follow up ratios, length of stay)
  • Provide training and supervision to junior doctors.
  • Provide training and support to specialist frailty practitioners.
  • Participate in rolling teaching programme.
  • Engage fully in consultant appraisal with a view to achieving revalidation.
  • Provide support/sessions in ED if required.
  • There is no on-call included in this post.
