Job summary
This is an exciting opportunity to join the Urology team at Southport & Ormskirk hospital sites .
The post holder will be an integral part of the Urology team, who assist, support and co-ordinate the pathway for patients who have completed primary treatment of prostate cancer.
A large part of the role is providing administrative and coordination support to patients, families, consultants, cancer nurse specialists and other staff involved in the cancer pathway.
The successful applicant will ensure the efficient channelling and close co-ordination of the patient follow-up pathway ensuring robust communication betweenmedical staff, GPs, patients, relatives, members of the public and other disciplines within the Trust and close liaison with the community staff andoncology teams.
You will need to be able to demonstrate excellent communication, interpersonal, organisational and IT skills and be willing to undertake ongoingtraining and development.
We welcome applications from caring, motivated healthcare assistants, experience with Oncology and Urology patients would be an advantage but not essential.
Main duties of the job
The post holder will work closely with the Urology CNS team to deliver outstanding care to the prostate cancer patients already registered and established on the remote risk stratification scheme.
They will be expected to provide patients and carers with emotional, physical and spiritual support during their cancer journey.
They will be expected to arrange the patients blood tests, collate results, collect health questionnaires, set up virtual clinics and then liaise with the CNS team.

- Be the point of contact for all routine enquiries facilitating communication and information flow, initiating and responding to correspondence and resolving enquiries.
- Organising incoming and outgoing mail both papers and electronic.
- Maintain an accurate database of patients including systems to track interval investigations and monitor incoming health status information from patients using the system.
- Liaise with GPs/cancer services as appropriate.
- Liaise with patients and other consultants secretaries.
- Specifically assist the CNSs in co-ordinating care by tracking patient pathways and providing a point of access, including rapid re-entry into the system for those people identified as having urgent or specialist needs.
- Triage incoming calls, using a risk assessment framework and initiate appropriate response according to protocols and individual pathways.
- Using good communication skills, and appropriate tools and procedures, liaising as appropriate with CNS when non-routine and refer complex decisions to the team for assessment and review.
- Provide basic telephone advice and refer on or signpost to other sources of support.
- Liaise closely with the MDT co-ordinator ensuring robust systems of notification and where appropriate co-ordinate transfer of care.
- Co-ordinate/act independently to make OPD appointments for those with abnormal results, fast track patients back into the system if required, and to respond appropriately when faced with a sudden deterioration or an emergency situation by alerting other members of the team.
