Mersey and West Lancashire Teaching Hospitals NHS Trust are working in collaboration with Cheshire and Merseyside Cancer Alliance (CMCA) to ensure that all patients with a cancer diagnoses have access to personalised care interventions .Personalised Care and Support Planning is a core deliverable of the NHS Cancer Plan and ensures that patients and their families are supported to live with and beyond their cancer diagnosis
The aim of this project is to ensure that Personalised Care and Support Plans (PCSP) (based on Holistic Needs Assessments (HNAs)) are available for all cancer patients, and data is submitted to Cancer Outcomes and Services Data set (COSD).
Personalised care has been identified as a key priority within the NHS Long Term Plan and the aim contributes to the overall Long-Term Plan personalised care ambition that:
- Where appropriate every person diagnosed with cancer will have access to personalised care, including needs assessment, a care plan and health and wellbeing information and support.
Main duties of the job
The successful post holder will lead on a digital personalised care project They will lead on developing Standard Operating Procedures (SOP) / Policies for HNAs and PCSPs (linked to LCN CQG Personalised Care Working Group outcomes - HNA Guidance / Principles). The post holder will lead on:
- Relationships and communications with the Trust Informatics Department, Macmillan, MWL cancer nurse specialists, cancer support workers and clinicians at all levels
- developing a plan to improve the quality of data compliance for personalised care interventions across Trust
- the roll out the e HNA digital platform for holistic needs assessment
- the work with the BI team to develop a interactive dashboard on all Personalised care interventions deliver a wide range of clinical systems such as SCR and My Care plan
- reporting on MWL and site-specific team performance against KPI's for the CMCA board and COPOG
- developing a trust SOP for Personalised Care interventions
- the development of a suite of KPI's with clinical teams to demonstrate activity and quality of PC interventions
- the development of sustainably reporting tools
- optimising the interface with SCR and EDMS of e HNA platform
- improving performance on the number patients using e HNA platform electronically
- handover solution

Job responsibilities
The role will be undertaken by an experienced nurses who will lead the stakeholder engagement, communications and training required by the clinical teams to ensure that systems are adopted, implemented in a timely manner and that benefits are achieved, and patient outcomes are improved.
Responsibilities extend into Business as Usual (BaU) to monitor the systems for compliance and act accordingly and escalate concerns, as necessary, to senior members within the Informatics team and the Hospital Trust. The role will provide hands on clinical advice to users so they can use the systems effectively for the benefit of patients and staff. The role will also ensure that the changes and rollout of the above systems are completed safely in line with Trust priorities and policies.
KEY DUTIES
- To be the link between Informatics and the Trust, ensuring effective communication between Informatics and Clinical subject matter experts.
- To engage Trust clinicians at all levels and ensure implementation of the IT projects and to be a point of contact for system-related problems.
- To be visible, supportive, and responsive to resolve any queries.
- Lead on the pilot implementations of clinical systems, and subsequent rollout across all specialties working closely with the clinical leads.
- To champion the timely updates of the system across the Hospital, such as real-time Admissions- Discharges and Transfers (ADT) so that patients appear on the correct ward lists
- Lead on the implementation of clinical systems and their subsequent rollout across the Trust working closely with the operational clinical leads in all care settings.
- To jointly lead on the IT end user acceptance testing, to ensure the system is clinically sound and any changes introduced meet Trust policy.
- To review and amend changes to Trust policy as required.
- To provide advice for the Projects Team, ranging from clinical advice to arranging staff to access training.
- To lead and advise the necessary training programme for to ensure that clinical users are trained to a suitable standard.
- To provide opinions based on clinical experience.
- To champion the use of the systems and its benefits realisation.
- To monitor the systems for compliance and act accordingly to address issues of non-compliance. To then diagnose the reasons and escalate concerns when appropriate to senior members within the trust.
- To work closely with the Clinical Leads and keep them informed of all major developments and discuss the system regularly.
- To attend the Project Boards/Project Team meetings where required and keep up to date with minutes and highlight reports.

ADMINISTRATIVE RESPONSIBILITIES
- To own the stakeholder communications plan and clinical aspects of the project website, providing timely responses to clinicians, escalating where appropriate
- To run the ADT compliance report monthly. To target areas and implement strategies to help improve the Trusts ADT compliance.
- To monitor compliance the compliance of systems in line with hospital and national policy and take corrective action in areas of non-compliance.
- To ensure that mobile technology deployed in clinical areas is suitable for use in the operational settings and complies with Trust clinical and infection policies.
- To keep the link nurse register up to date and ensure this is communicated to the trainers and the help desk staff to ensure staff can be trained by link nurses and have passwords as needed.
- To run the Task and Finish Group and Users Groups. To organise and plan each meeting including the agenda. They are responsible for the distribution of the previous meetings minutes and agenda prior to the meetings as well as the organising and running of the group.
- Be responsible for Standard Operation Plan as well as developing the clinical content of the intranet pages. To ensure that anything that is amended in the SOP is updated appropriately in the training manuals so that all information is consistent.
