An exciting opportunity has arisen for a dynamic and enthusiastic registered nurse to join the award-winning Diabetes and Endocrinology Team in the role of Diabetes Specialist Nurse
The Diabetes Specialist Nurse will be part of a wider multidisciplinary team which has a strong commitment to improving standards of diabetes and endocrine care locally. We are looking for an individual with a passion for diabetes who is motivated, willing to develop their knowledge and drive-up standards of care locally.
A significant part of the role will involve working autonomously providing inpatient diabetes care and education at the trust hospital sites. Also supporting the diabetes emergency phone line and providing support to local health care professionals and outpatient services as needed. Individuals will need to be flexible and able to work shift patterns/weekends. Training and support will be provided to successful applicants.
Main duties of the job
The Diabetes Nurse Specialist will work as a member of the Diabetes and Endocrinology Multi-Disciplinary Team providing specialist nursing expertise to patients and health care professionals within an inpatient, outpatient or community setting. They will be expected to advise secondary and primary care clinician's, maintaining links across organizational boundaries, acting as a source of specialist knowledge working within agreed protocols and guidelines.
They will act as an autonomous practitioner providing nurse led care to patients requiring diabetes and endocrinology specialist intervention in several areas for example structured staff and patient education, insulin initiation or medication titration advice. They will order and undertake diagnostic tests for patients with diabetes and endocrinology conditions and have continuing responsibility for the assessment of care needs, their development, implementation and evaluation, setting high standards of care at all times.
They will develop and deliver specialist education for nursing, medical and other care health professionals involved in the care of adults with diabetes and endocrinology conditions to ensure an effective high-quality service, raising the profile and understanding of diabetes and endocrinology care within and across the trust. They will maintain expertise through specialist supervision, on-going training,research and audit providing evidence based practice to improve the quality of diabetic patient care.

- Participate in review of diabetes inpatients and or nurse led clinics using relevant protocols whilst exercising judgement, discretion and decision making in clinical care.
- Discuss complex patients or those for specialist investigations with a senior member of the diabetes and endocrinology team as appropriate.
- Liaise and support primary and secondary care clinicians with diabetes and endocrinology clinical care and management, including supporting the diabetes emergency phone line on a rota-based system which will be accessed by health care professionals and patients.

- Support patients and their families during diagnosis of diabetes and treatment ensuring understanding of their diabetes and any related complications

CLINICAL & PROFESSIONAL RESPONSIBILITIES

- Autonomously assess triage and diagnosis diabetes and endocrinology conditions and review those with longstanding diabetes/endocrine conditions as inpatients or outpatients, recommending or initiating appropriate treatment within agreed protocols and guidelines.
- Order, undertake, interpret and act upon diagnostic tests and results for patients with diabetes or endocrine conditions
- Assess, plan, co-ordinate and implement and evaluate evidence-based diabetes care in conjunction with relevant health care professionals.
- Undertake a number of specialist diabetes procedures which will include management of diabetic inpatients, delivering diabetes education, insulin initiation and titration, oral hypoglycaemic initiation and titration, managing patients with technology and diabetes.
- Effectively manage own workload.
- Involve patients, carers and relatives wherever possible in the planning, implementation and delivery of diabetes and endocrinology specialist care.
- Undertake nurse prescribing if considered relevant.
- Ensure compliance with specific regulations in regard to diabetes/endocrine drugs and administration and record keeping within area of work.
ADMINISTRATIVE RESPONSIBILITIES
- Ensure compliance with specific diabetes protocols, standards and clinical paperwork including patient experience satisfaction and diabetes related surveys.
- Undertake and provide telephone consultation/support as appropriate to patients and health care professionals.
TEACHING & TRAINING RESPONSBILITIES
- Provide specialist knowledge, advice and support to colleagues, health care professionals, patients, carers and relatives around diabetes and related conditions.
- Contribute to the multidisciplinary education and training provision/programme for health care professionals involved in caring for patients with diabetes across organisational boundaries.
- Develop co-ordinate and participate in diabetes/endocrine education and training programmes and evaluate their effectiveness.
- Raise awareness of diabetes and related conditions through health promotion activities and organised events throughout the trust and wider community.
- Provide support and education to patients, allowing them to develop self-management skills as appropriate to their diabetes.
- Provide verbal and written advice to patients and carers with diabetes/endocrine conditions, around care management as required in hospital and in preparation for discharge.
- Facilitate an appropriate learning environment for students and junior staff providing direct supervision,mentorship, training and evaluation to students and new starters within the trust.
- Participate in regular clinical education sessions within the diabetes unit.
- Identify, contribute to, or lead on some aspects in the development and dissemination of evidence-based guidelines, standards and protocols as required.
