Job summary
Great opportunity to gain experience during maternity leave within medicines management (3 days per week), whilst maintaining a clinical specialty on wards (2 days per week). This is a temporary contract for 12 months until the postholder returns from maternity leave.
We are seeking a motivated pharmacist to work within our friendly medicines management team, supported by the Lead Pharmacist and another pharmacist. The extensive pharmacy clinical service highly respected and recognised as a key enabler by the Trust, supporting improvements in patient flow, whilst providing high quality patient care and experience.
You will provide support to the professional and operational aspects of medicines management, including decision-making for new medicines and clinical guidance; and the management of systems to support safe and effective non-medical prescribing, and administration of medicines under patient group directions.
Main duties of the job
Provide a highly specialised clinical pharmacy service to the agreed specialty. This includes but is not limited to:
- Taking drug histories, completing medicines reconciliation and screening patients for medication related issues as soon as possible after admission
- Following up patients post discharge from hospital, ensuring changes to patient's medications are communicated to the GP and patients are aware of any changes to their medication
- Assessing the patient's need for medication and determining the optimal choice of medication to meet that need, ensuring that any potential negative interactions, contraindications / cautions etc. are identified and communicated effectively to the responsible clinicians
- Provide highly specialised pharmaceutical advice to clinicians, other healthcareprofessionals and patients, both within the Trust and externally as appropriate
- Demonstrate advanced levels of clinical reasoning and judgement, and use persuasive skills when communicating with medical staff - highlighting the reasons underpinning the medication choice to ensure recommendations are accepted. In circumstances wherethere is a conflict of opinion, the pharmacist is responsible for ensuring that the consultant responsible for the care of the patient, is aware of the evidence under-pinning the recommended treatment choice, and is in a position to make an informed judgement aboutthe most appropriate treatment
