Job summary
An opportunity has arisen for an enthusiastic, motivated and experienced nurse with excellent communication to join the Diabetes Team. We are recruiting to a part time diabetes transition specialist nurse working closely with age groups between 16 and 25 years.You will work closely with the Consultants, Clinical Nurse Specialist's and other multidisciplinary team members playing an important role in the delivery of care and management of people with diabetes both in the community and acute setting. Based between Bentley Medical Centre and Manor hospital you must be able to work autonomously supporting this population across the borough of Walsall , organize your workload effectively and adapt to, as well as drive change at a local level, being flexible is key. You should be a registered nurse or desirable registered children's nurse with extensive post registration experience in nursing. A post graduate qualification in diabetes or experience in diabetes clinical care would be preferred.

Main duties of the job
- To work autonomously in the provision of specialist, evidence based diabetes care with a focus on the 16 to 25 year olds
- To provide a high quality community service for people with diabetes within Walsall Healthcare NHS Trust.
- To assist in the implementation and adoption of local agreed diabetes policies and guidelines
- To maintain Walsall Healthcare NHS Trust Standards in all aspects of the job.
To support transitional patients ensuring innovation is maximised in promoting engagement
About us
Walsall Healthcare NHS Trust is an integrated Trust and the only provider of NHS acute care in Walsall, serving a population of 286,700, providing inpatient and outpatient services at the Manor Hospital as well as a wide range of services in the community. Walsall Manor houses the full range of district general hospital services under one roof. The £170 million development was completed in 2010 and the continued upgrading of existing areas ensures the Trust now has a state-of-the-art Critical Care Unit, Neonatal Unit, Obstetric Theatre, and Integrated Assessment Unit facilities. Work is also well underway on its multi-million pound new Urgent and Emergency Care Centre which will house a new Emergency Department (including Children's Emergency Department), co-located Paediatric Assessment Unit, Acute Medical Unit and Urgent Care Centre. This is scheduled for completion this year.
