Job summary

The post holder will support the Respiratory Nurse Specialist in providing a Respiratory Service that is evidence based and provides seamless care following agreed care pathways.

The role will include assessment of patients with respiratory disease and the development of individualised management plans in conjunction with the medical team. The provision of Education to the respiratory patient is essential and the post holder will have excellent communication skills. The post holder will be expected to develop excellent links with primary care.

The post holder will possess a Respiratory disease qualification

Post holder will act in a manner that upholds the values of the Trust and that of the profession ensuring the service delivered contributes to supporting the principles of clinical governance.

Main duties of the job

Under the direction of the Respiratory Nurse Specialist and guidance of the Respiratory Medical team to promote early supported discharge of COPD patients, discharging patients safely and assisting in reducing lengths of stay. Adhering to national guidelinesThe post holder should ensure the needs of all respiratory patients are recognised and met by the assessment, planning, implementation and evaluation of care. To participate in MDT with the respiratory team and promote care management to ensure the patients journey is seamless.To provide advice and support to patients undergoing nebuliser assessments and maintain the hospital nebuliser loan service.To provide advice and support to ward staff regarding all aspects of oxygen therapy and working in accordance with Walsall Healthcare's oxygen policy and the BTS oxygen guidelines.To participate in the pulmonary rehabilitation programme by providing education sessions to small groups of respiratory patients. To also promote the service by referral as per the patient discharge bundle.To participate in the Xolair severe asthma clinic by administering injections and assessing the patients who attend and liaising with heartland hospital to maintain the patients ongoing care.To participate in the teaching programme to educate and develop other qualified staff including students on an ongoing basis on the wards as required. To ensure the sharing of clinical and theoretical knowledge to promote research based practice.

About us

This is an exciting opportunity for an experienced registered nurse to join this specialist nursing service and gain expert knowledge in respiratory nursing. You will be working as a key member within the respiratory multidisciplinary team. The post requires vision, innovation and motivation with the ability to work across professional boundaries, underpinned by a real desire to provide the highest standards of patient care.

You will gain knowledge in all respiratory conditions such as Asthma, COPD, ILD and Bronchiectasis. You will assist in the assessment of respiratory patients and understand the role of the National Asthma and COPD Audit Programme. We hope at the end of this exciting secondment opportunity you will have gained knowledge that will enhance the care of the respiratory patient.

It is essential that you possess up-to-date relevant clinical knowledge and have had some experience in respiratory either on a dedicated ward environment or in the acute setting. You must demonstrate evidence of ongoing continuous professional development. You must be highly motivated with strong clinical, ability to work as key member of the team and possess excellent communication skills.

CLINICAL RESPONSIBILITIES

  4. To participate in MDT with the respiratory team and promote care management to ensure the patients journey is seamless.
  5. Liaise with multi - disciplinary and multi - agency working in order to achieve the best possible outcome for patient care.
  6. To provide advice and support to patients undergoing nebuliser assessments and maintain the hospital nebuliser loan service.
  7. To provide advice, assessment and support to patients and ward staff regarding all aspects of oxygen therapy and working in accordance with Walsall Healthcares oxygen policy, the local HosAR and the British Thoracic Society oxygen guidelines.
  8. To participate in referral to the pulmonary rehabilitation programme, To also promote this service as per the patient discharge bundle.
  9. To be able to provide smoking cessation advice and recommend interventions and support.
  10. To observe the Xolair severe asthma clinic by accompanying the respiratory nurse in administering injections and assessing the patients who attend and liaising with tertiary sites to maintain the patients ongoing care.
  11. To have knowledge of the National Audit programme and complete the audits in a timely manner.

  13. To observe service improvement and participate in sharing new ideas and changes in clinical practice.
  14. Maintain the Trusts aims and objectives within the clinical setting.
  15. Accept accountability and responsibility for accurate and legible record keeping in accordance with local policies and NMC guidelines.
  16. Undertake additional training as required to meet the needs of the patients and register any additional skills acquired as a result of enhanced practice.

MANAGEMENT RESPONSIBILITIES

  1. Assist in the efficient and effective use of equipment and resources to meet the needs of the service.
  2. Ensures effective communication with the multi-disciplinary team.
  3. Assist in maintaining a safe level of nursing staff to allow the the respiratory nurse service to provide continuity of care. Assist in taking remedial action and report to senior management when this cannot be achieved.

HEALTH AND SAFETY

  1. Under the direction of the Respiratory Nurse Specialist and others, contribute to clinical leadership activity through the generation and implementation of policies and practice guidelines, developing care plans and or care pathways where appropriate, auditing these and taking remedial action when required.
  3. In conjunction with the Respiratory Nurse Specialist contribute to the audit and benchmarking of clinical care within the division of medicine and long term conditions and the Trust.
  4. Adhere to the trusts complaints procedure with regard to written and verbal complaints, using appropriate referral mechanisms in order to resolve the situation. Will audit and highlight to the team and senior managers particular deficits in relation to complaints and where appropriate necessary remedial action is required and/or in conjunction with PALS team.
  5. In addition, individual staff have a role in suggesting and implementing approved improvements to service(S) and in exercising professional responsibility for both themselves and their peers within an open fair and just culture.

CONTINUING PROFESSIONAL DEVELOPMENT

  1. Assist in the assessment and continual professional development of clinical staff using mechanisms such as Individual performance reviews to generate a personal development plan in accordance with Trust policies and procedures.
