Main duties of the job
We are recruiting to a full time diabetes transition specialist nurse working closely with age groups between 16 and 25 years.
To provide care in both the acute and community setting for people with diabetes including clinics in GP surgeries, health centres and patient homes
To work within a multidisciplinary team alongside consultants, nurses, podiatrists, dietitians and educators
To deliver both patient and health care professional education sessions in accordance with the service level agreement.
About us
You will work closely with the Consultants, Clinical Nurse Specialists and other multidisciplinary team members playing an important role in the delivery of care and management of people with diabetes both in the community and acute setting. Based between Bentley Medical Centre and Manor hospital you must be able to work autonomously supporting this population across the borough of Walsall , organize your workload effectively and adapt to, as well as drive change at a local level, being flexible is key. You should be a registered nurse or desirable registered childrens nurse with a minimum of 5 years post registration experience in nursing. A post graduate qualification in diabetes or experience in diabetes clinical care would be preferred
