Hepatocellular Carcinoma Surveillance Pathway Navigator

Job summary

An exciting opportunity has arisen for a crucial admin role as a Pathway Navigator for patients on Hepatology Cancer Surveillance and will be the central point of contact for the patients on HCC surveillance; helping to ensure the individual needs of the patient are supported and that care is built around those needs.

This is an administrative role and will be part time. It will be approximately 18 hours, this can be discussed with the suitable candidate. This post could be considered for remote working (from home) with a requirement to work on site for at least one day a week.

This role is not availablefor Skilled Worker visa sponsorship. Applicants must already have the legal right to work in the UK at the time of application. Unfortunately, we are unable to support visa applications for this post in line with UKVI regulations.

Main duties of the job

By being the Pathway Navigator within the Hepatology team for Dorset County Hospital NHS Foundation Trust, you will provide direct support for patients to maintain their engagement in the surveillance pathways ensuring that they receive their surveillance scans on schedule. Working closely with the Hepatology CNS and Consultant lead for Hepatology.

As the central point of contact you will be involved with maintaining both effective communication with and on behalf of the patient, signposting to other resources and information as defined by patient need and liaising with staff and services to ensure tests and appointments are scheduled.

A key element of the role will be to maintain and populate tracking lists for Liver Health Check clinics and to track patients for HCC Surveillance, as such, you must have excellent IT and data entry skills.

  • NVQ or apprenticeship level 3 in Health
  • Numeracy skills and literacy skills at level 2 (or equivalent)

Knowledge & Experience

Essential

  • Proven experience in a caring role
  • Understanding of acute hospital environment and the role of a pathway navigator
  • Previous experience of working with the general public
  • Experience of using a computer

Desirable

  • Experience working with diverse audiences (e.g. volunteers, patients and/or community groups)
