Job summary

Are you a nurse with experience of caring for children and young people (CYP) with diabetes and would like to lead a team of diabetes nurses with a focus on leading on digital technologies for the service. We are seeking an autonomous practitioner with leadership skills and extensive experience of caring for CYP with diabetes.

Main duties of the job

Main responsibilities will include:

o The assessment, management and evaluation of treatment and care for CYP aged 0-18 years with diabetes

o Leading and co-ordinating a team of paediatric diabetes nurses.

o Acting as an expert resource in Paediatric Diabetes with extensive specialist knowledge and clinical expertise to be an effective educator for health care professionals.

About us

The Paediatric Diabetes Team is a small MDT team who provide high standards of care for approximately 100 CYP with diabetes. Currently co-located on Paediatric Unit, the team have close links with the inpatient ward, Paediatric Assessment Unit and medical team, providing experience and leadership in all aspects of paediatric diabetes nursing care across the paediatric workforce.

The post holder will provide specialist diabetes nursing care, working across inpatient, outpatient and the community setting throughout the West Dorset locality.

Job responsibilities

  • An autonomous practitioner, alongside the Paediatric Diabetes team, including clinical lead and medical colleagues, responsible for the assessment, primary and/or secondary diagnosis, management and evaluation of treatment and care for a caseload of children & young people aged 0-18 years with diabetes in both the hospital and community.
  • The post holder will provide nursing care for a defined caseload, educating and supporting the child/young person and their family/carers in diabetes self-management skills from diagnosis to transition.
  • Co-ordinate and implement an overall strategy of evidence-based care for the whole caseload of patients
  • Co-ordinate the service alongside the Clinical Lead and lead practitioners for medicine, nursing, dietetics and psychology.
  • Jointly lead and line manage the Paediatric Diabetes nursing team.
  • Be a highly visible and accessible resource for specialist advice to professionals and carers providing care to children and young people with diabetes.
  • Through effective leadership, ensure the clinical multidisciplinary team deliver a safe, effective, responsive, caring and well-led service where compassion is embedded and a positive patient experience is achieved.
  • Provide strong professional clinical and managerial leadership and act as a professional role model.
  • Works across organisational and professional boundaries, including working collaboratively with health care professionals in the community and external agencies, for example social care, education and the voluntary sector.
  • Act as an expert resource in Paediatric Diabetes with extensive specialist knowledge and clinical expertise to be an effective educator for health care professionals, CYP, their families and carers (including the education setting).

Qualifications & Expereince

Essential

  • Registered Child branch or Registered Sick Children's Nurse (RSCN),
  • Educated to BSc Degree level in relevant field
  • Recognised Post graduate Paediatric Diabetes Qualification
  • Significant demonstrable experience within paediatric diabetes care with evidence of professional development within this specialism.
  • Minimum 5 years post registration
  • Leadership development programme or willingness to undertake leadership training
  • Evidence of developing policy, guidelines and managing resources
  • Independent Non-Medical Prescribing qualification (Active on the NMC register) or willingness to undertake
  • Car owner with valid driving licence

Desirable

  • Previous experience in line managing staff
  • Teaching qualification or equivalent
  • Experience of working collaboratively with a range of other services within primary, secondary and tertiary care, social care and education
