Job summary
Richmond Response and Reablement Team (RRRT) are excited to be recruiting for a passionate individual to work on the Urgent Community Response pathway. Regardless of Nursing, Paramedic or Therapy background, we are looking for a registered and experienced healthcare professional with assessment and prescribing (V300) skills and a key interest in avoiding unnecessary admissions to hospital.
RRRT is a seven-day integrated health and social care service for adults, primarily older people, working in partnership with HRCH and the London Borough of Richmond upon Thames Council.
Main duties of the job
The post holder will be supported by the 8a Advanced Clinical Practitioner to work autonomously and alongside the MDT in assessing patient needs, initiating investigations and appropriate treatment, and providing their clinical leadership to support colleagues.
