Job summary
The key purpose of the role is to support and lead on patient safety incident investigations, key aspects of the risk management, patient safety and quality improvement agenda across the Trust to create an organisational patient safety and risk aware culture that focuses on high quality care standards, preventing avoidable harm, avoidable death, and embedding mitigation by using evidenced best practice, lessons learned and data to identify incident themes, and areas for improvement.
The post holder will promote patient safety, lead the PSII's, support safety learning and contribute to the development of organisational learning and improvements in clinical risk and patient safety at service line and organisational level. This will be done by promoting a culture in which reporting of incidents and use of PSIRF strategies is routine practice and is recognised by staff as a means of improving the quality of clinical care and reducing risks.
Main duties of the job
1. Using strong leadership, communication, influencing and negotiating skills lead in the provision of expert advice, support and guidance to the organisation to ensure that learning from incidents makes a positive difference to patient safety.
2. To contribute to improving the management of patient safety in the Trust by developing and supporting strong recommendations that will lead improvements.
3. Identify, instigate and lead on / participate in specific quality improvement projects related to patient safety.
4. To o v e r s e e the robust and effective application of Patient safety Incident Investigations and PSIRF reviews through the management of the Patient Safety and Risk Managers. This will include, but is not limited to the identification and declaration of PSIIs, update of StEIS, and co-ordination of PSII round tables, investigation meetings, Duty of Candour, Staff support, report writing and safety recommendations.
5. To support with PSIRF training throughout the organisation.
6. To be the Trust expert on PSIRF
