We are seeking Rehab Assistants Practitioner to work on a temporary, as-needed basis.
We are seeking a motivated and dynamic Rehabilitation Assistant Practitioner with a passion for general physical rehabilitation to work within NELFT's London based Intensive Rehabilitation Service (IRS).
The IRS is a large multidisciplinary community team which offers an alternative to hospital bed-based rehabilitation, supporting patients to recover at home within their own environment. As part of the team within IRS, you will have a caseload of community patients and you will support the Therapy Team in IRS to provide patient-centred treatments.

Main duties of the job
- The post holder will work as a Rehabilitation Assistant Practitioner, supporting the delivery of highquality strength and balance interventions through routine clinical assessments, exerciseinstruction, and patient-centred care across the designated localities.Key Responsibilities include:o Triaging referrals into the Falls Strength and Balance Exercise Service to ensure patientsmeet the agreed eligibility criteria.o Conducting initial face-to-face home assessments to determine suitability for either anOtago-based home exercise programme or a Postural Stability Group Exercise Programme.o Leading and delivering structured strength and balance home-based or group exercisesessions in accordance with service guidelines.o Ensuring full compliance with Trust policies, including health and safety requirements, loneworking procedures, infection prevention and control, and completion of all mandatory riskassessments.o Supporting the ongoing development, evaluation, and improvement of the Falls Strength andBalance Exercise Service.o Where appropriate, contributing to the development and assessment of competencies forBand 3 unregistered support staff.o Providing clinical supervision, mentorship, or task-based support to Band 3 unregisteredstaff, if applicable.o Travelling efficiently across multiple locations using own car to deliver care and fulfil servicecommitments
