Job summary

We are seeking a motivated and dynamic Band 5 Community Nurse to join the Specialist Children's Community Nursing Team in providing excellent quality nursing care for children with Complex Needs attending Special School in Waltham Forest.

The Specialist Childrens Community Nursing team comprises of three teams - Child Development Nursing, Special School Nursing, and Childrens Community Nursing team. offers care to children across Waltham Forest in their homes, in clinic and in Special School settings. The aim is to provide an excellent standard of Nursing care to enable children to achieve their best possible outcomes.

The post holder will assist the existing Children's Community Nursing Team in carrying out a range of clinical activities. They will utilise a range of skills and experience gained in clinical practice to provide holistic assessments for children who have complex needs. Clinical judgement will aid the postholder to formulate a care plan based on these assessments. The postholder will also demonstrate excellent time management skills to enable them to work autonomously at times. Excellent communication skills are essential as well as good knowledge of current Nursing best practice and the ability to work as an effective team member across the Specialist Childrens Nursing Teams as well as the wider Multidisciplinary Team and external agencies.

Main duties of the job

  • To be directed by the team lead or shift coordinator in prioritising day to day work and case management
  • To work as part of the Community Children's Nursing Team to ensure the ongoing assessment, planning, implementation, and evaluation of individualised evidence-based programmes of care.
  • To provide high quality, evidenced based nursing care to children, young people within their home, school, nursery, or other community setting - working at all times within the limits achieved of the competency framework for the Community Children's Nursing Team
  • To work in partnership with the child, young person and their family and carers, to identify individual needs and deliver nursing care. To encourage and support self management and independence of clients and families, providing training and support within limits of identified competence.
  • To be responsible for accessing annual mandatory training in line with trust policy. To be responsible for ensuring attendance at clinical supervision and monthly one to one with preceptor/supervisor meetings
  • To identify limits of own knowledge/skills and seek advice as and when necessary, from team members or specialist practitioners - or other members of the multi professional team.
  • To work as part of the Community Children's Nursing Team to provide support in discharge planning from acute care to community settings, including children with continuing care needs.

About us

Starting with NELFTNELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

This post attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,551 and a maximum payment of £5,735 per annum pro rata).
