Job summary

The post holder will have the NNEB, Health care assistant or equivalent qualification and have experience of working with children with developmental delay. The post holder will be responsible for patients allocated to them and will support the team and school staff in identifying the child and family's need and providing support and delivering care and support families/carers,

The post holder will support the family being able to sign post children/YP, families/carers to available resources within the borough for additional support and information.

The post holder will assist the team members, paediatricians and the wider multi-disciplinary team to contribute to the management/assessment of children/young people (YP) with special/additional needs, (including weight/height management and measurement)

The post holder will assist, in the assessment of health needs, planning, and providing care for both individuals and groups of school-age children attending Special Schools. The post holder will be an advocate for the child and family both within the service and the wider team.

The post holder will establish and maintain effective communication with children, young people, parents and carers, colleagues, multidisciplinary team, and LA to achieve a positive health status for the child and family.

Main duties of the job

Key Responsibilities:

To be responsible for organising own workload whilst supporting the team in the day-to-day allocation of work.

To implement agreed interventions and health promotional developments for children with additional needs supporting the child/YP & family to achieve full potential.

To have knowledge relating to trust protocols and procedures and adhere to them, particularly confidentiality.

To support the Clinical Nurse Specialist and team in providing accurate records of information required by the trust for audit purposes.

To ensure effective risk management at team level by accident/incident reporting, and ensuring any identified risk is communicated to the Clinical Nurse Specialist and team for further action

To participate in multi-disciplinary review meetings for complex children Looked After Children, providing reports and information as appropriate.

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will includemandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focussing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

Probationary Period

This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

High Cost Area Supplement

This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,551 to a maximum of £5,735).
