Are you a dynamic and compassionate physiotherapist looking to make a significant impact on patients' lives ? .We are seeking a skilled and dedicated band 7 Physiotherapist to join our Integrated Health and Social care teams for a 1 year fixed term project .
We are excited to pilot a new project in Waltham Forest for patients with double handed care following discharge from acute hospitals or those in the community.
The role involves working jointly with Occupational Therapists Rehabilitation assistants and social care colleagues to provide timely assessments and provide equipment/ interventions aimed at reducing the level of care for patients who have double handed care / high care needs.
Main duties of the job
You will
- Work collaboratively with health and social care professionals across the borough and Integrated Care System.

- Be proficient in advanced clinical skills and knowledge with the ability to assess the physical and psychosocial needs and develop a comprehensive and concordant plan of care with the patient, their carers and the wider MDT as needed.
- Work collaboratively and carryout joint visits members with MDT colleagues within the team to promote holistic person-centred assessment and approach to care
- Reporting patient outcomes using agreed reporting systems
- Support and mentor pre registration learners from a variety of discipline

High Cost Area Supplement - Outer London (B&D, Redbridge, Havering and Waltham Forest)
This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,313 to a maximum of £5,436).
