Job summary

The post holder will be a Community Learning Disability Support Practitioner responsible for patients allocated to them and will assess, manage, plan and deliver care, including being professionally and legally accountable.

The post holder will support their peers and team leader and be an effective team member whilst working on their own initiative. They will have a commitment to collaborative working and actively supporting and liaising with other health and social professionals and agencies.

  • To provide a high standard of care within their scope of practice as delegated by the senior nurse on duty.
  • To provide care to patients in a range of settings in the community. This may include nurse-led clinics, patient's homes, Residential Homes and hospitals
  • To work under the supervision of a Registered Nurse and continue to develop and undertake a range of delegated tasks independently.
  • To assume responsibility for their workload ensuring that they adhere to NELFT policies and procedures.

It is anticipated that this role will continue to develop through the acquisition of further skills, knowledge and competencies

Main duties of the job

  • To manage a client caseload with regular supervision and be a named person for clients which involves co-ordinating the care of the client, compiling reports and reviews.
  • To work autonomously in prioritising caseload, adjusting working day as necessary, making decisions without direct supervision.
  • To assist the Community Nurses in identifying clients' primary and secondary health care needs and support to access the appropriate resources
  • To work in conjunction with the Community Nurses to implement individualised health care plans.
  • To assist in the assessment, planning, implementation and evaluation of health plans within a multi-disciplinary framework.
  • To work with clients in carrying out a range of tasks with them which will help meet their physical and mental health needs and assist them to develop their skills and competencies associated with their health needs.

About us

Starting with NELFTNELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

Probationary PeriodThis post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).High Cost Area SupplementThis post also attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,108 and a maximum payment of £5,177 per annum).
