Job summary
The post holder will have a central role in providing medical input within the newly formed community Mental Health & Wellness Team, an integral part of the transformation programme of community mental health services in the borough. The new team places a strong emphasis on systemic, trauma-informed working and continuity of care.
Duties will include assessing and reviewing patients in clinic/with home visits, devising treatment plans, clear documentation and correspondence with the relevant colleagues to ensure plans are actioned. You will work closely with the other members of the multidisciplinary team (including nurses, social workers, occupational therapists, psychologists and peer-support workers) to ensure that patients are provided with the highest standards of care.
Main duties of the job
The post holder will have a continuing responsibility for the care of patients in their care,whilst undertaking administrative duties associated with patient care.To be active members of the Multi-Disciplinary Team and to attend weekly Complex Cases meetings and the referrals and allocations meeting.Where appropriate, participate and play a leadership role in the assessment and management of cases with severe and enduring mental health disorders.To play a role in the interface with primary care networks, ARRS potholders, social services, and peer support workers toacross neighbourhood teams when neededTo provide liaison with the inpatient medical team for patients, including attending CPA and discharge planning meetings.To provide in-service training and supervision to junior doctors and other non-consultant-gradedoctorsMulti-Disciplinary Team members.To contribute to the teaching of medical students and trainee doctors.To participate in weekly teachingTo be part of Clinical Governance, including participation in audit and quality improvement
About us
The team works with people with a range of mental health problems from moderate psychological difficulties (e.g. mood/anxiety disorders which cannot be managed in Primary Care) to those with severe and enduring mental illnesses who require care coordination. You will have weekly supervision with one of the Consultant Psychiatrists in the team.
