Job summary

Waltham Forest ICT Central integrated community team has an exciting opportunity for a Band 4 Nurse Associate/Assistant Practitioner to provide care and to assist clinicians in Waltham Forest Community.

The progressive role will allow the post holder to be part of a dynamically diverse team in the hub of East London providing a high standard of care

Main duties of the job

  • The post holder will be a member of an Integrated Team in Waltham forestthat enable patients, their families and carers to spend more time actively involved in pursuing the identified goals on the care plan in their own home.
  • The post holder is expected to work with minimal supervision across a wide variety of environments.
  • To work collaboratively and in support of all statutory health, social care and voluntary sector providers, primary and secondary care professionals, specialist services, users and carers

Probationary Period

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

High Cost Area Supplement - London

This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,313 and a maximum payment of £5,436 per annum pro rata).
