We are recruiting Primary Care Network Mental Health Practitioners for the Redbridge area.
A Primary Care Network (PCN) is a group of GP practices that work together in a local geographical area. We are recruiting for one vacancy in the Wanstead and Woodford PCN.
The role has been co-designed by our Mental Health clinicians and GP colleagues, with the aim of improving patient access to mental health care and support. As an additional resource based on the patients' GP surgery, your role would include assessing patients' needs, signposting them to support resources, referring to other services and, where appropriate, offering brief intervention.
The post-holder will be employed by NELFT and will be a member of the local Mental Health service team, supported by a line manager and peers, but will be based in a Primary Care Network working in Wanstead and Woodford (WW) PCN premises.
This is an exciting time to join the team as we are developing relationships and building links between Primary Care and local Mental Health and Wellness Teams. About us
