Job summary

Urgent Community Response Team delivers innovative community health and social care crisis management, with a view to admission avoidance based on the vision of the NHS Long Term Plan. We aim to reduce the number of attendances to A&E and hospital admissions as well as care homes. The team works with paramedics, Adult Social Care Staff, Nurses and AHP Therapist.

Main duties of the job

  • To act as an autonomous, registered practitioner who is legally and professionally accountable for own unsupervised actions guided by the professional code of conduct and Trust guidelines and protocols.
  • To be able to assess and develop plans of care to meet the complex needs of patients with a variety of conditions. This will include continuously evaluating and acting on outcomes. To provide patients and relatives with information and education thus ensuring they have meaningful choices that promote dignity, independence, and quality of life.

The post-holder will have or be prepared to undertake the following duties:

  • Be able to assess the unwell patient
  • Take observations
  • ECG recording
  • Phlebotomy
  • Male/ Female Catheterisation
  • Suprapubic Catheterisation
  • Involvement in Chronic Disease Management
  • Management of constipation
  • Referrals to other appropriate services- e.g. Social Services, Specialist Nurses
  • Mentoring students/ new staff
  • Teaching HCA's, students, patients and carers
  • Auditing/ Benchmarking
  • Developing IT skills/ documentation

Probationary Period

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

High Cost Area Supplement - London

This post also attracts payment for High Cost Area Supplement of 5% of the basic salary (minimum payment of £1,192 and a maximum payment of £2,011 per annum pro rata).

The post holder will support their peers and team leader and be an effective team member whilst working on their own initiative. They will supervise pre-registration students. They will have a commitment to collaborative working and actively support and liaise with other health and social care professionals and agencies.

To be a valuable member of the Urgent care team and make a contribution to patient care and team working. To function as a Primary Nurse without direct supervision whilst working alongside the clinical leads, who maintains ultimate accountability for the service. To assist in leading and monitoring the performance and work of the nursing team. The aim of this role is to undertake the responsibility for care as required and deputise for the team lead as and when required.
