Job summary
An exciting opportunity has arisen for a motivated, enthusiastic Band 3 Rehabilitation Assistant to join our Discharge to Assess (D2A) Therapy Team.We are seeking a caring and dedicated Rehabilitation Assistant who has already worked within a health and / or social care setting. Previous NHS experience and qualifications in a therapy or healthcare related subject would be desirable.Appropriate patients are discharged from hospital settings and placed within the D2A therapy-led care home beds for up to six weeks. The purpose of this position is to assist the therapy team to provide assessment, rehabilitation, management and discharge of patients. Therefore, moving and handling of patients and equipment is an essential part of the role.In addition, the candidate must be IT literate and will be expected to carry out routine administrative and housekeeping tasks when required.Patients will be seen across designated care homes, therefore the post holder will need to have a full driver's license, as well as access to a car. Main duties of the job
- To work as a Rehabilitation Assistant within the Discharge to Assess (D2A) Team, working closely with the Therapy Team within designated D2A Care Homes, Local Authorities and BHRUT.
- To be responsible for planning, organising and prioritising own workload in the day-to-day allocation of work, including being professionally and legally accountable, under the direction of the Therapists.
- To be responsible for providing regular Therapy support in partnership with the care home providers to maximise patient outcomes.
- To be responsible for monitoring treatment programmes and feeding back information as directed by qualified Therapists. Interventions may be individual, or group based.
- To participate in regular reviews in line with the D2A guidance under the direction of qualified Therapists.
- To be able to manage the therapeutic handling of patients often with the need for prolonged physical effort.
- To assist the qualified Therapists by liaising with other health professionals, agencies including Social Services, Local Authority colleagues, voluntary and independent sectors, families and carers.

Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process, new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.
High-Cost Area Supplement - London
This post also attracts payment for High-Cost Area Supplement of 15% of the basic salary (minimum payment of £4,313 and a maximum payment of £5,436).
