Job summary

An exciting new role has been developed within the Redbridge Integrated Care Directorate for a Senior Referrals Coordinator . This post is open to qualified mental health practitioners of any discipline, including nursing, occupational therapy and social work.

The post-holder will work across the three Mental Health and Wellness Teams operating to provide borough-wide mental health services. These new multi-disciplinary community mental health teams were developed as part of the Mental Health Transformation and include a range of traditional disciplines, and new roles implemented as part of Transformation. They work closely with statutory and voluntary partners, especially Primary Care and the Primary Care-based mental health practitioners. They have strong connections with inpatient and acute services including home treatment teams, specialist drug and alcohol services, day service providers and psychological therapies. The Mental Health and Wellness Teams provide all secondary adult mental health services in the borough and are based on Recovery, Open Dialogue and Trauma Informed models of care. They offer assessment, advice and signposting to other teams and organisations as appropriate, and individual interventions and groups for service users aimed towards recovery and independence.

Main duties of the job

  1. To prioritise own day-to-day workload.
  2. To support admin staff to manage referrals identified as urgent or high-risk.
  3. To ensure urgent or high-risk referrals are screened and offered triage or further follow up as necessary within appropriate timescales according to level of risk.
  4. To lead the daily referrals screening meeting to ensure a robust clinical screening process within timescales in line with local and national targets.
  5. To provide clinical guidance to the duty staff and triage practitioners, particularly in relation to clinical risk management of new referrals.
  6. To ensure walk-in assessments are seen in a timely manner and robust risk assessment completed.
  7. To liaise with and support MHWT clinical leads to manage the assessments allocated to their teams
  8. To identify any system, recording or process issues with the management of referrals into and out of the MHWT triage services and implement actions to address these
  9. To offer support and guidance to other staff members and services as appropriate.

As a senior clinician they will play an active role in the ongoing development of the service, working alongside the Mental Heath and Wellness Teams seniors and supporting the service managers. They will need to have excellent clinical and risk assessment skills, goodcommunication and leadership skills and an ability to work autonomously, prioritising and managing their own workload as required.

We are looking for an enthusiastic, flexible individual who has a passion forsupporting mental health service users and for developing the skills and knowledge of multi-disciplinary staff. Although there are some opportunities for agile working, due to the nature of the job, on-site location is expected for the majority of the role.
