Job summary
An exciting opportunity has arisen at the Hillingdon Hospital NHS Foundation Trust to recruit four Macmillan Cancer Support worker's to work across a number of services (Urology, Breast, Colorectal, Lung & Upper GI).
The Macmillan Cancer Support Worker will form an integral part of the multidisciplinary team in caring for the physical, social and psychological needs of patients diagnosed with cancer and their carers throughout their cancer pathway.
The Support Worker will work with the Clinical Nurse Specialists (CNSs) to ensure the coordination and management of patients at all points of their treatment. The Support Worker will act as a point of contact for patients under the guidance of the CNS to signpost patients based on their needs. Support workers will work independently within the remit of the role under the supervision of the Clinical Nurse Specialists and Personalised Care Project Manager.

Main duties of the job
- A key aspect of the role involves daily and direct (e.g. face to face and telephone) communication with patients, relatives, carers, and other health and social care professionals. Communication in this context requires a high degree of empathy, understanding, diplomacy, honesty, and integrity.
- To provide general information and support about cancer and cancer services, to enable people to navigate the health and social care system to make informed decisions.
- Provide information to patients related to treatment/follow-up and signpost to appropriate healthcare providers within remit of role and guidance.
- Perform Holistic Needs Assessments using designated screening tools as appropriate.
- Support CNSsin developing and facilitating Health and Well Being Programs. This will involve contributing and delivering education to patient and carers, including information around health promotion, disease prevention and supporting self-management of their rehabilitation or long-term condition.

Identify with the CNS patients requiring a Holistic Needs Assessment and support those requiring Level 1 psychological support within own skill base.
Complete routine HNA on pre-identified patients in clinic, on the telephone and in the cancer information Centre, and escalate their concerns to CNS where appropriate.
Organise own workload, ensuring the CNSs is updated of progress in line with agreed work plan.
Monitor and check appointments and diagnostic tests are booked and undertaken in an appropriate and timely manner.
Ensure patients receive timely and appropriate information in regard to their diagnosis and treatment and signpost to appropriate supportive leaflets and literature.
Respond to telephone queries and make appropriate referral to CNS or other allied healthcare professional, and to act as a point of contact to patients.
Communicate directly with patients, relatives and carers demonstrating empathy, understanding and respect. Information discussed and received will often require tact and clarity, to ensure safety and understanding.
Participation in obtaining and collating patient satisfaction with care using questionnaires, results of Friends and Family Test and new patient experience tools.
Participation in the development and support the planning, delivery, and evaluation of Health and Wellbeing events in collaboration with the survivorship support worker.
Maintain stock levels of relevant Macmillan & local support information within the clinical areas.
Be aware of the particular physical and psychological needs of the patient, observe, listen and report on any changes to the patients condition or concerns, alerting the CNS or registered nursing staff so that any necessary treatment and/or reassurance can be given to maintain patient safety/well-being.
Plan and organise own workload to ensure patient needs are prioritised, which will also involve liaising with several other healthcare professionals and external agencies.
