Job summary

The Senior patient flow Coordinator is responsible for actively supporting the discharge and care at home for patients with any diagnosis in the last weeks of their life. This is to enable a timely and safe discharge from hospital to the Persons' preferred place of care be that home, hospice, or care home.

The post holder will identify patients admitted to Hospital who are in receipt of Fast-Track Continuing Health Care (CHC), which will help manage the length of hospital stay between decision to discharge and actual discharge. Reduce the number of patients who do not need or want to remain in hospital for end-of-life care by facilitating a robust discharge plan and a responsive, well-coordinated, communicated discharge.

Whilst the initial focus of the Role will be to promote and support the patients that are under the care of the Palliative care team, there will also be instances when the ward consultant processes a fast-track pathway for a patient which will also need to be collaborated and supported.

The Senior Patient flow coordinator will also be expected to support junior colleagues and provide a level of expertise in the discharge process. This will involve quality checking and standardising the documentation, ensuring all junior colleagues are informed of how the complete tasks and follow up with them to ensure that the task is completed.

This Role will act as a direct supervisor for the PFC's working closely with the other senior PFC to support the Patient flow coordinators to have a high quality of data inputting by ensuring that the data inputted is of the same high standard. This also involves ensuring that the Patient flow coordinators hand out the discharge booklets and inform the patients, carers and families of the discharge plans.
