Job summary
Band 2 Working with the Frail elderly from 65 and above.
1. The healthcare assistant / support worker will Provide patient centred, compassionatecare at all times and advocate for the patient, they will assist the Patient's to maintain asmuch independence as possible whilst in Hospital.2. The healthcare assistant / support worker is a member of the team that delivers clinicalcare under the supervision of a registered nurse. Tasks are delegated by the registerednurse.3. The post holder is responsible for ensuring that patients are cared for in a safeenvironment and that privacy and dignity is maintained at all times.4. The post holder will provide direct care and provide verbal and written reports.5. To actively seek the views of patients / carers on their understanding of their care needsand develop a relationship to ensure the patients experience is positive.6. The Healthcare Assistant is required to successfully complete all 15 standards within theInduction Care Certificate and associated workbook within the set timeframe of 4 weeks(maximum 12 weeks), if they have not completed it previously.

Main duties of the job
Ensuring they deliver high standard of care ensuring.
1. To assist the registered nurse / professional in the delivery of individualised patient care2. To effectively communicate with patients, in such a way, that respects and takes into account the patient's level of understanding.3. To assist with the preparation and delivery of meals and beverages, assisting patients as requested.4. Adhere to strict compliance with Trust Infection Control policies and procedures.5. To participate in the admission process of patients as directed by the registered nurse.6. To assist the registered nurse / professional in the delivery of patient care - pre, peri and post procedures, as required within scope.7. To communicate effectively with patients / carers to ensure patients are kept well informed of any changes or delays to appointments / investigations.8. To be responsible for undertaking and recording blood pressure, pulse, temperature, respiration, urine analysis, weight, height, reporting any abnormalities to the nurse / professional in charge.9. To help patients within the parameters of their level of mobility, using equipment as required, ensuring risks are minimised.
