Job summary
This is an exciting opportunity for an enthusiastic and dynamic individual to further develop their career with our established Hepatology Team. The team were proudly recognised in the Trust's 'Outstanding Team Work' award category and were selected to present their work on the development of the hepatology service at the European Association for the Study of the Liver in 2020.
The successful applicant will help the team to continue its development and manage the increasing number of outpatients and inpatients with chronic liver disease. The post holder will help to manage a cohort of outpatients with liver disease and will support the development of a service in-reach, out-reach, early supported discharge, and admission avoidance.
The post is Band 6/7, dependent upon experience. For Band 6 level applicants, Band 7 would be applied once you begin undertaking clinics independently (with supervision).
The post will be based across the Trust's sites. There may be a requirements for out-reach at Addiction Recovery Community Hillingdon (ARCH) in Uxbridge.

Main duties of the job
The post holder will play an integral role in the development of a quality driven service to all patients at/or admitted to the Trust with liver disease. You will have a leadership role within the Trust and will be responsible for the development of innovative nursing practice within the department. You will also be expected to facilitate the development of other nurses in order to benefit patient care. You will be expected to be an educational resource to all members of the Trust-wide multi-disciplinary team regarding practice provision of care.
The post holder will help to promote and coordinate a seamless pathway of care for patients with liver disease, from diagnosis through to treatment and outpatient management. Integral to the role is a collaborative approach with the multidisciplinary team to provide a comprehensive and patient-focused service.
