We are looking for someone with health and wellbeing coach skills and experience in diabetes working with the diabetes team . See job description for further details. This is a fixed term contract for 7 months.

Diabetes care is provided to patients in the form of structured education, in community-based clinics and in their own homes throughout the London Borough of Enfield.

The successful candidate will work with the Diabetes community service, GP practices and voluntary organisation. The Health and Wellbeing Coach will play an important role in engaging patients and use health coaching techniques to support and empower them to take an active role in the self-management of diabetes. You will work with patients of low to medium complexity that may have one or more long term conditions.

Main duties of the job

Health coaching is a partnership between health and care practitioners and people. It guides and prompts people to change their behavior, so they can make healthcare choices based on what matters to them. It also supports them to become more active in their health and care. Helping people gain and use the knowledge skills and confidence to become active participants in their own care; to achieve self-identified health and wellbeing goals.

Health and wellbeing coaches predominately use health coaching skills to support people with lower levels of patient activation to develop the knowledge, skills, and confidence to manage their health and wellbeing, whilst increasing their ability to access and utilise community support offers. They may also provide access to self-management education, peer support, and social prescribing.

As a Health and Wellbeing Coach you will be a key member of the MDT, working with the diabetes. You will work with patients holistically, considering the whole person. Health coaching revolves around using coaching skills to support people with lower levels of patient activation to develop the knowledge, skills, and confidence to manage their health and wellbeing, whilst increasing their ability to access and utilise community support offers. Your role and skills will support and encourage the prevention of developing further illness, or the deterioration of existing long-term conditions.

  • BSc in Nutrition / Health Physiology / other Health coaching / other qualification / training
  • Understanding of the importance and process of helping people with long- term conditions to develop their knowledge, skills and confidence in managing their health and the range of models and tools available, such as the Patient Activation Measurement (PAM) Experience delivering group sessions / group talks
  • Experience of using coaching approaches/frameworks and models or other helping strategies e.g. Motivational Interviewing. Experience delivering group sessions.
  • Good IT skills and Knowledge of Microsoft applications
  • Experience of working in a team
  • Skilled in active and reflective listening, building trust and rapport quickly
  • Full UK driving license and own a car
