Job summary

  • Band 6 Health Visitor: Specialist Community Public Health Nursing (SCPHN)
  • Based in the Enfield Paediatric Liaison Health Visiting Team, NMUH.
  • Hours: 37.5 /week.
  • Responsible to the Team Leader.
  • Accountable to 0-19 Community Services, Service Manager.

Main duties of the job

  • Triage, review and documenting the A&E attendance cards for Children 0-19 years old, and dissemination to professionals in hospital and the community.
  • Research and information collation.
  • Processing received hospital Safeguarding and Antenatal referrals, documenting on RiO and sharing information as appropriate
  • Attend Multi-Disciplinary Meetings: Safeguarding, Maternity, Paediatrics.
  • Prompt response to enquiries from Health Visitors, School Nurses, Hospital staff.

About us

We are a specialised team based at North Middlesex University Hospital, who respond to the attendance of Enfield Children attending the Emergency Department. Last year the Enfield PLHV Team received approximately 33,000 attendance cards to review.

have close connections with the Hospital Safeguarding Children and Safeguarding Midwives Teams. We liaise with the four Health Visiting Teams and the School Nursing Team, and also the Looked After Children Nurses, Youth Offending Service Nurse and Special Schools Nurse Team.

The Community division comprises of the 0-19 Service, Health Visitor and School Nursing Services that deliver the Healthy Child Programme. As well as a BCG Clinic for Targetted babies and Canterbury ward, that supports the acute wards.

Job responsibilities

The post holder will work within the Enfield Paediatric Liaison Health Visiting Service, to provide timely patient information sharing, identifying the most vulnerable patients and families to their community practitioners and hospital safeguarding.

  • Daily Monday -Friday triageing of seven days a week A&E attendance cards for children 0-19 years old and then sharing it as appropriate with the 0-19 service, and other services as required: Hospital Safeguarding Children Team, out of area Health Visitor/School Nurses teams, country wide LAC Teams, Youth Offending Service, Oasis Youth Work group.
  • Working with and supporting the Paediatric Liaison Team.
  • Accurate and timely entry of data.
  • Ensuring client records are kept safe, confidential, up to date and accessible.
  • Liaising with internal and external agencies to ensure that all necessary information is provided in an efficient and timely manner.
  • Attending and participating in meetings. e.g. Monthly Team meetings and Professional meetings as required.
  • Using a variety of IT systems to review, record and send out client information, such as: NHS.net email, RiO, CiP, Medway/Careflow, intranet, Internet and Microsoft packages as required.
  • Supporting the Paediatric Liaison Health Visitor team, advising and supporting colleagues.
  • Ensuring security of the premises when leaving.
  • Handling both routine and urgent matters using initiative and with minimal supervision.
  • Undertaking any other duties commensurate with the level of the post as required ensuring efficient and effective running of the department/section.
