The successful candidate will be responsible for co-coordinating the administrative, physical and psychological care of patients attending clinics for assessment.

We are looking for a motivated and enthusiastic Health Care co-ordinator to join our team. This post will play a crucial role in ensuring quality and safe care is maintained, whilst moving forward to meet the evolving needs of the service and population we serve.

Main duties of the job

The main duties of the role are to ensure that the patient receives a high standard of care in the department

You will need:

Excellent communication skills

Good practical skills and organisational ability

An understanding of the pivotal role Pre Assessment has in the inpatient pathway

Good IT skills

Ability to work on own initiative and as part of a team

About us

  • We are a small team located in the outpatients department.
  • We work closely with our Anaesthetist colleagues and have good working relationships with all of the MDT members ensuring the patient's pathway is seamless.

Job description

Job responsibilities

Undertake assessment of identified patients through the triaging process ensuring fitness to proceed to surgery.

Responsible for taking and recording basic observations

To locate, track and prepare skinny files

To provide assistance to medical and nursing staff as and when required

To take an active part in Pre Assessment meetings and discussions

Phlebotomy

To help maintain a high standard of patient care, remembering that we are often the first contact that patients and relatives have with the hospital

To prepare consulting rooms, clinics and trolleys

To assess patients that have been triaged as part of the POA process to ensure they are fit for admission.

To make appointments and cancel appointments

To record patient attendances at clinic, entering data on the computerised Patient Administration System (PAS)

To ensure patients who do not attend (DNA) are dealt with according to Trust guidelines

To print clinic lists and labels, filing forms and specimen bottles

Responsible for taking and recording basic observations

To notify the Medical services Transport Controller once patients are ready for collection

To ensure equipment and furniture is in good working order and to report loss, damage or need for replacement to the Senior Nurse

  • Experience of working within a healthcare environment for 2 years
  • Have good administration and organisational skills

  • NVQ level III in Care or equivalent
