Job summary

The Discharge Nurse will be responsible for facilitating safe andeffective discharges of complex patients from hospital to a community setting through a multidisciplinary approach. They will co-ordinate assessments of patients and support the discharge plan.The Discharge Nurse will have a caseload of the most complexclients and will be responsible for completing and coordinating assessments and discharge plans for this group of patients, ensuring that all parties are updated at all times.This group of patients Includes but is not exclusive toHomeless patients,ensuring that the correct pathway is followed and that thetrust are in receipt of any changes to legislation or the processComplex patients requiring either specialist rehabilitation orplacement ensuring that the correct pathway and funding streams are applied for and adhered to incorporating any changes to legislation or process.Overseas patients with no access to public funds requiring intervention from the Home Office or repatriation to their country of originThe postholder will be responsible for providing specialist advice and support to the wards and the MDT's in managing effective discharge. They will take the lead in Family and Professionals meetings providing guidance and support to both professional colleagues and patients/ families

Main duties of the job

Active input and support to the MDTs for each ward and ensure feedback on actions assigned.To facilitate effective multi-disciplinary discharge planning and processes as an integral part of care management throughout all the divisions.Demonstrate a high level of clinical judgement and act autonomously Tracking and assistance in completion of referrals where necessary, of allpatients referred to ensure supporting referrals are completed in a timely and efficient way and that they progress on to the next stage of their pathway as soon as possible.Ensure that patients that are going to another provider for care, do so with the right information, consumables and equipment.
