Job summary
The post holder will provide high quality front line administrative support to the complex discharge team. This role involves working closely with Nurses, IDT and Flow coordinator . The post holder will work as part of the complex discharge team ensuring an efficient, high quality and timely service is provided. This includes providing a welcoming and professional service to patients, relatives and all other services i.e. IDT and community partners.
The post-holder must play a proactive role in collecting, investigating, verifying, and recording a high standard of patient demographic and treatment data. The post holder must accurately and in a timely manner enter such information on the computerised patient information system, to ensure that the trust receives the appropriate level of income for patient activity, avoids the potential clinical risks inherent with inaccurate patient records and is compliant with national NHS targets (e.g. on data quality and length of patient wait).
Due to the nature of the role, the day-to-day duties of this post, the post can be unpredictable, non-routine and deal with time pressured situations.
Main duties of the job
To provide administration service for complex discharge team directing as appropriate.
To demonstrate sensitivity, tact and diplomacy where appropriate, for example when acting as a first point of contact via email or phone calls to community partners and hospital.
To answer the telephone and take messages, answer or refer queries as appropriate. Answering phone calls/redirecting, taking messages when staff unavailable.
Daily monitoring of generic complex discharge team inbox and responding to emails from internal and external parties
To provide a confidential and high standard general administrative support to the complex discharge team.
supporting the service with tasks such as discharge spread sheets and audit tool completion.
Be trained and conversant in Cerner to ensure that all patient discharge summaries are available.
transport booking are recorded accurately on Cerner or the relevant electronic system in real time.
To arrange transport internally (via the patient transport booking system) and externally (couriers) for patients and equipment when the need arises, in line with the trust's transport policy
Screen all pre booked transport for accuracy of transport booking.
