Job summary

The Community Neurological Conditions Management Team (CNCMT), based out of the Edgware Community Hospital are looking for a full time Band 3 Rehab Assistant, with a passion and interest to support people living with Neurological Conditions. The post holder will work with adults in their own home to support with increasing independence and improving quality of life goals.

As part of this role, you will be required to work as member of the therapy team within the CNCMT service. You will be joining a motivated, caring, inclusive and compassionate team. You will work with a team of Therapists to set goals and assist patients to regain their independence as well as develop skills to manage aspects of their condition.

Responsibilities will require you to work independently within the community with regular support and supervision of the clinicians within the team. Previous rehabilitation/care experience, working in the community or on a ward setting would be desirable.

Main duties of the job

Implement planned rehab and therapy sessions for individuals in their own home under the guidance of qualified MDT members

Actively participate in learning opportunities to maintain knowledge and inform practice. Actively seeks feedback.

Manage own workload and time, for example maintaining own diary.

To assist clinicians in administrative tasks required for the efficient day-to-day running of the team.

Accurate record keeping of clinical documentation.

To attend mandatory and statutory training as identified at appraisal and by trust policy.
