Job summary
To lead a team of clinical pathway navigators and adminsitrators and be responsiblefor the smooth running of the patient journey from referral to discharge from Trust careby ensuring that coordinated and streamlined administrative processes revolvearound the patient and their individual needs.The post holder will also be expected to carry out duties as per discussions with theService Manager, Assistant Operations Manager and Operations Manager. The postis a challenging job in a supported and structured environment
Main duties of the job
This involves developing strong working relationships and working closely withnavigators, clinical pathway administrators and clinical team PAs within their team andother managers, all key members of the multi-disciplinary team within primary andsecondary care and service users relating to the service specialty they cover to ensureall patients receive a consistent and effective service. This includes working withconsultants, medical, nursing and therapy teams, divisional and operationalmanagers, central appointment office, ward administrators, diagnostic and imagingdepartments, external agencies e.g. GPs, other hospitals, social services, police etc
