Job summary
This is a two-part role comprising of both Vascular Access (VA) and Hospital @Home (H@H) Services. The role sits in the Nurse-led Vascular Access (NLVAS) & OPAT service.
The main purpose of this role is:
- Providing a responsive and comprehensive H@H service facilitating safe discharges into the community for patients requiring intravenous (IV) therapy and maintaining Antimicrobial Stewardship (AMS) in the Organisation.
- Providing a reactive and comprehensive patient centred VA.
Main duties of the job
The H@H is a new comprehensive elective service to adult in-patients and outpatients at The Royal Free London NHS Foundation Trust. The base site for the post is the Royal Free Hampstead Site and the post is expected to cover across sites.
The post holder will be dynamic practitioner with an intense passion for Intravenous Therapy, patient training and facilitating Outpatient Parenteral Antimicrobial Therapy (OPAT) discharges. Proficiency in OPAT modalities and processes is essential, and the post holder must have experience of working in OPAT. The post holder will have responsibility for complication management and ensuring best practice is adhered to throughout the organisation.
The post holder will support the Lead Nurse in the day-to-day management of a complex cohort of patients requiring H@H and Vascular Access and assist the Lead Nurse in implementing strategies:
to reduce clinical risk
for informal and formal training & education
Clinical audit.
The post holder will work in collaboration with the H@H case finder to facilitate continuing care and safe discharges.
This is a 12-month fixed-term contract, with secondment opportunities available for internal NHS candidates.
