Royal Free NHS Foundation trust is recruiting Band 4 Nursing Associate / Band 4 Assistant Practitioner in our Barnet Renal Dialysis Unit.
We are looking for an Nursing Associate/ Assistant Practitioner with substantial experience in Haemodialysis and successful completion of either Haemodialysis competencies or equivalent to work as a member of a Nurse led Dialysis unit. As an Nursing Associate / Assistant Practitioner you will work as part of the team accountable for the delivery of patient care under the supervision of a registered nurse.
You will be proficient and have experience of initiating & terminating dialysis including cannulation of AV Fistulas. You will have experience in training haemodialysis patients to perform self-care. Your responsibilities will include preparing equipment for treatment, ensure it is kept clean & supplies are well-stocked, safely stored, used efficiently; keeping documentation up to date, provide safe quality care, ensuring the best clinical outcomes and patient experience.
You must be educated to foundation degree in Health & Social Care or equivalent and have recent experience of working with patients in Haemodialysis area. You will be a good Multi-disciplinary team worker with flexibility. Main duties of the job
Contribute to the provision of a safe and therapeutic environment for allpatients, relatives, friends and hospital visitors.To ensure high standards of care are given to all patients, monitoring this throughspot checks and clinical quality indicators outcome measures.To contribute to patient and relative feedback by undertaking daily clinical roundswith the aim of providing excellent standards of care.To collaborate in the assessment of patient care needs and the development,implementation and evaluation of plans of care, ensuring high standards ofevidence based care.To ensure communication with patients and significant others is established,ensuring empathy and reassurance regarding, assessment, care and treatment.To understand and support safeguarding principles for patients and escalateany concerns to the registered nurse.To facilitate early discharge planning being carried out to enable appropriatearrangements to be in place for patients leaving hospital or transferring in order toachieve a safe and timely discharge from hospital.To ensure that all patients are cared for in a clean and well-maintained clinicalarea, ensuring that all staff are aware that they are directly responsible forcleanliness standards when in charge and escalating issues when necessary.
