Job summary
The post holder will work collaboratively within the Transformation Partners in Health and Care to ensure effective management of interdependencies across the portfolio of programmes and engage extensively with external key stakeholders including senior clinicians, senior managers in ICBs, SPGs and NHS England (London).
The post holder will:
- The post is responsible for commissioning a pan London complex specialist service for vulnerable people.
- The post holder will have specialist expertise in order to define current processes, facilitate discussions and advise colleagues as to how best services might be adopted in the definition of future processes, document those processes as they are agreed and oversee their delivery.

Main duties of the job
Main duties of the role:
- Contract monitoring for Find and Treat service delivery in 23/24.
- Reviewing current outreach services for prevention, detection and management of communicable disease cases and outbreaks in inclusion health populations provided across London, and how these services are commissioned and funded. In consultation with key stakeholders identify strengths /weaknesses in the system and service provision.
- Using available data and evidence establish an overview of the needs of inclusion health populations in London.
- Develop and test proposals for service and funding model and commissioning arrangements.
- Working with partners, agree next steps including who will lead 24.25 commissioning
