The Children's services at the Royal Free London are committed to providing excellent care to our children, young people and their families.

Royal Free London is one of the providers for North Central London as a result we cover a large geographical area including Camden, Islington, Barnet and Enfield. We have a number of bases and the applicant will need to be flexible which site they work.

We are looking for an enthusiastic, highly motivated, dynamic and forward thinking professional to join our well-established Paediatric diabetes nursing team who are committed to enhancing and developing their service. Previous experience in diabetes

As a member of a multidisciplinary team the successful applicant will demonstrate a level of expertise within the specialist service, providing specialist advice, education and support to healthcare professionals, schools and voluntary organisations as well as parents/carers. They will have a key role in the management of these children in the community or referring hospital. You will be supporting children, young people and their families from diagnosis through to transition to adult services.

This is an ideal opportunity for career progression for a paediatric nurse with experience in looking after children with diabetes.

Main duties of the job

JOB SUMMARY:

The Diabetes Specialist Nurse will work collaboratively with all members of the Multidisciplinary team that constitutes the Diabetes service at the Royal Free London Trust to deliver effective, high quality, safe patient centred care on their main Hospital site, and in the community setting. They will also liaise with and support the Diabetes team members on other Royal Free London Trust sites to ensure a standardised approach to Diabetes care delivery and service development. She/he will contribute to the continuing education, training and professional development of the nursing staff to promote the acquisition of new skills and knowledge in the clinical areas. She/he will support the MDT to deliver best practice for all patients and their families driving forward a programme to ensure excellent care delivery.

The Diabetes Specialist Nurse will:

  • see children on the in-patient ward, ambulatory ward, outpatients and A&E and at home in the community
  • be the focal point in the service for the Children, young people and families working closely with them from diagnosis
  • work with the children, young people and families to ensure a smooth transition to adult services
  • support the children, young people and families through different treatment pathways
  • support self-management of their diabetes
  • liaise closely with the multidisciplinary diabetes team and other professional

The post holder will cross cover the Diabetes Specialist Nurses on other Trust sites as required

CLINICAL RESPONSIBILITIES

  • Act as an advocate for the children, young people and their families.
  • To develop a nursing service within the Childrens Diabetes service by receiving direct referrals.
  • To undertake nurse lead clinics to review children with diabetes
  • To develop treatment plans for patients, families and schools in a format agreed with the multidisciplinary team.
  • Demonstrate appropriate expert practice and specialist advice in the assessment, planning, implementation and evaluation of childrens diabetes care demonstrating high levels of autonomy and advocacy.
  • Manage a caseload of patients, as appropriate within the speciality, delivering individualised patient care working collaboratively with the diabetes Paediatrician.
  • Provide clinical expertise, specialist advice and support across the service including the diabetes clinic, home visits, school visits, ward attenders, minimising admissions whenever clinically appropriate.
  • To support clinical staff requiring mentorship/clinical support in relation to patients individualised care.

  • Work closely with the Diabetes Paediatrician and other multi-disciplinary team members, anticipating their information needs, providing timely data, using own analytical skills.
  • Initiate and adjust insulin dosages as considered appropriate under protocols/nurse prescribing agreed with the Diabetes Paediatrician, working as an independent practitioner within said protocols.
  • Demonstrate in depth specialist knowledge, skills and innovative practice in childrens diabetic care delivering care and advice in a variety of settings including hospital, the childs home and school.
  • Establish effective communication channels within the multidisciplinary team and with healthcare workers/professionals in neighbouring Trusts/Primary Care settings and schools.
  • Ensure the completion of patient records and support the maintenance of the Twinkle database.
  • Actively promote infection prevention and control awareness using best practice to reduce the potential of cross infection.
  • Promote and adhere to lone worker procedures ensuring personal safety when practising within the community setting.
  • To undertake clinical care to maintain expertise in childrens diabetes practice in line with NMC Rules & Standards.
  • To be aware of and work in accordance with NICE guidance.
  • To teach children and their families using evidence based practise ensuring that the teaching is relevant and up to date. Ensure teaching programmes are tailored to meet the needs of the family.

  • Communicate with children, young people, their families and carers, providing reassurance and assisting in developing their problem solving skills. Ensure that complex and sensitive information is communicated in an appropriate language that is clearly understood.
  • Promote patient involvement and enhance their experience by developing new initiatives and supporting patient centred care and self /group support.
  • To support childrens parents and other carers as partners in care. Teaching clinical skills as required.
  • Monitor childrens diabetes care standards, taking corrective action when procedures or services fall below the required standard gaining support from senior medical and nursing staff as and when required.
  • Ensure child protection procedures are adhered to and the needs of the child remain paramount in all environments.
  • Deal with a variety of complex circumstances, including distressed and anxious children, adolescents and families, school staff, and relatives by working closing with Child and Adolescent Mental Health service and social services as required.
  • To travel between paediatric service sites, external organisation and patients homes as required on a frequent basis.
  • To support children in the community by liaising with other agencies and offering teaching and advice.
