Main duties of the job
Royal Free London Hospital Trust (RFL) is seeking a Band 8A Healthy Living Hub Programme Manager to deliver an effective, high quality integrated Healthy Living Hub service. The post holder will lead the identification of health improvement priorities and the design, project management and implementation of these for the service.
The aim of the new integrated Healthy Living Hub is to support and increase focus on prevention and a reduction in inequalities across North Central London (NCL). There will be a particular focus on ensuring that the prevention priorities within the NHS Long Term Plan (LTP) that related to NHS Trusts are implemented, starting with smoking cessation initially and building from there.
This will be delivered through brief behavioural interventions; supporting patients to stop smoking, manage alcohol consumption and achieve weight loss for health. The service will use evidence-based interventions, including active referral to community services and direct provision of services within the Trust to achieve this.
As Healthy Living Hub Manager you will work across the RFL Respiratory Team and Public Health Team. However, it is expected that you will spend a significant amount of time working with other internal and external stakeholders from across NCL including but not limited to, VCS partners, local authority public health teams and the NCL CCG.
