Job summary
An opportunity for a Cardiac rehab lead has become available within the MSE Foundation trust.
As part of the role you will be responsible for the Cardiac rehab service based at southend hospital and although already established you will help promote further ongoing development and continue to move the service forward.
You will also support and manage the cardiac rehab team and help promote good relations with all members of the MDT across the MSE site, working closely to establish good working relationships and processes.
We are looking for candidates who have relevent experience working within a cardiac rehabiliation team, with proven leadership skills , who have the passion and enthusiasm to lead the cardiac rehab service at the southend site.
Main duties of the job
- To act as an information resource for patients, carers and staff.
- To establish good relationships with patients and their families through all phases of cardiac rehabilitation
- To facilitate telephone follow up/pre assessment calls and ensure accurate documentation.
- To hold a patient case load for the Cardiac Rehabilitation exercise programme and take responsibility for the patients care and discharge.
- To compile comprehensive and accurate advice and discharge letters to Gps and other multi-disciplinary staff.
- To manage Nurse led clinics according to service protocols and guidelines.
- To supervise and assist with the teaching and support of visiting students.
- To maintain a safe environment for patients attending cardiac rehabilitation as per risk assessments and protocols.
- To be aware and actively assess/ update all protocols and guidelines in relation to the cardiac rehabilitation programme.
- To assess, plan, deliver and evaluate patients care utilising an individual holistic approach.
- To monitor all relevant Nice Guidelines and make changes as appropriate.
- The jobholder will also carry responsibility for the planning of discharge from rehabilitation of cardiac patients including future goal setting, implementation of secondary prevention issues and informing primary care clinicians of patient's status and future needs.
