Job summary

The Cancer Clinical Support Worker will directly make the lives of patients with Breast Cancer better. This role forms an integral part of the multi-disciplinary team approach in providing personalised care interventions to patients diagnosed with cancer and their carers throughout the care treatment pathway and after care support.

The Support Worker will work independently within the remit of the role but under the guidance and supervision of the Cancer Clinical Nurse Specialists (CNS) and Personalised Care Team to co-ordinate and manage patients identified to the service by the CNS team.

Main duties of the job

The Cancer Clinical Support Worker will ensure patients are:

  1. Supported to regain as normal a life as possible
  2. Given advice about how to minimise their risk of developing further cancer related problems
  3. Given advice about possible future signs of cancer recurrence or long-term effects of treatment
  4. Enabled to re- access specialist services without delay should they need to do so (Improvising Outcomes: A Strategy for Cancer, 2011)
  5. Support the OAFU/PSFU clinics, and admin team, training will be given.

Job responsibilities

As part of a team, the cancer support worker will speak to patients to discuss what matters most to them through a holistic conversation and assessment. The cancer support worker will provide cancer information and/or signpost them to support services and resources and will develop a personalised care plan which is shared with the patient and specialist team and that this and other communications are documented on the Somerset Cancer Registry. Provide dedicated and knowledgeable support to patients receiving cancer care and treatment at UHB.

Provide basic telephone/face-to-face advice and triage where appropriate, working within defined protocols and guidelines. Document and communicate outcomes as per service specification.

Make pre-planned telephone calls to patients to assess on going needs to enable a proactive

prevention approach.

Support patients and carers to access timely and appropriate information and support, including sign posting to relevant departments or a range of support services.

Support patients and carers to understand what signs, symptoms or situations to be aware

of that would indicate concern.

Inform patients and carers on how to make contact when they feel that their condition or needs have changed, including what to do out of hours.
