Here is an exciting opportunity to join our Anticoagulation (Hematology) team as a patient Pathway Co-ordinator in Anticoagulation.
Clinical haematology is a collaborative and inclusive team based at Basildon Hospital. Our ambition is to deliver an excellent service to improve the health and wellbeing of our patients. We are looking for an enthusiastic person to join our happy team.

Main duties of the job
Work within a specific clinical area to facilitate the smooth running of the patient journey by ensuring that co-ordinated and streamlined administrative processes revolve around the patient and their individual needs.
Provide administrative support to all clinical staff within the clinical area including Consultants, Nurse Specialists and Junior Doctors ensuring effective co-ordination of their diaries etc. this will include cancellation and reinstating of clinics in line with planned and unplanned leave.
To provide cross cover for reception service to all visitors to Anticoagulation at Orsett hospital to ensure a consistent and effective administration service is maintained at all times.
Support the clinical team, Operational Manager and other Divisional managers in ensuring the speciality are able to deliver safe and effective services whilst also ensuring compliance with contractual access targets e.g. the 18 weeks Referral To Treatment target.
Provide cross-cover for other Patient Pathway Co-ordinators to ensure a consistent and effective administration service is maintained at all times.

Demonstrate high levels of customer care and be confident and empathetic on the telephone to patients as many will be hard of hearing.
Courteously and efficiently receive telephone enquiries from patients, carers and external agencies in relation to hospital appointments and general enquiries, ensuring that accurate records are maintained and appropriate action is taken in a timely manner.
Deal with patient requests and concerns promptly and courteously escalating concerns to the Anticoagulation nurses or specialist consultants.Close collaboration with Anti-Coagulation nurses.
To maintain patient records on EPR, EMR, DAWN and SystmOne. All training will be given.
Relay messages accurately from the nurses and on DAWN system to patients.Ensure pathways are validated throughout the patient journey and resolve / escalate concerns within the pathways accordingly e.g. risks to access targets etc.
To advise patients of INR results and inform of dosing instructions as set on INR Star.
